2️⃣ EMAIL photos/videos of your pet to us using the email button [BELOW], following these guidelines:

• • SEND PHOTOS/VIDEOS that clearly show your pet's face and any distinguishing marks.

  • Submit NO MORE THAN 5 files or 10 MB total.

  • Include your full name and your pet's name in the SUBJECT line.

2️⃣ PRINT and COMPLETE the form.

3️⃣ SCAN THE FORM using your scanner or a scanner app (e.g., Adobe Scan, Google Drive, or Apple Notes).
